The correct response should be

y=-6x+5.

This follow the form y=mx+b, b is the y intercept and m is the slope.

Slope intercept form y = mx + b where m = slope and b = y-intercept

Given : Slope = -6, y-intercept = 5

So equation in slope intercept form:

y = -6x + 5
